A comprehensive risk analysis of mooring operation of M/T Zarga is performed to enhance safety and mitigate risks in maritime industry. In this case, mooring operation had concluded with severe head injury of crew. Therefore, parted rope injury during ship mooring operation is selected as the TE since it may pose acute dangers for crew. The FT diagram for severe head injury during tanker mooring operation is provided in Fig. 2. The FT diagram is constructed under supervision ofsix marine experts.
