Post Natal ManagementLiver function tests should be repeated postnatally to ensure they revert to normal. Measurement should be deferred until approximately a week after delivery because in normal pregnancy there is sometimes an elevation in liver enzymes in the immediate postnatal period. Documenting postnatal resolution of liver function is important to secure the diagnosis of OC. A persistent elevation of liver function tests after the puerperium suggests an alternative diagnosis and referral to a hepatologist may be indicated. Clear communication of results to the GP and of the follow up plan is vital at the time of discharge from hospital.ContraceptionIt is generally recommended to avoid oestrogen-containing methods of contraception after a diagnosis of OC because of the risk of liver enzyme abnormalities. If this method of contraception is selected liver function should be monitored.Recurrence RiskWomen should be counselled that there is a high risk of recurrence of OC of approximately 80%.
