Purpose: The purpose of the DNP quality improvement project "The Use of a Flow Sheet to Enhance Referrals of Patients with Type 2 Diabetes for Diabetes Self-Management Education" was to increase referrals of patients with a diagnosis of Type 2 diabetes to Certified Diabetes Educators for diabetes self-management education. Background: Patients with Type 2 diabetes are more likely to reach and maintain their glycemic goals and be more satisfied with their care when they receive diabetes self-management education. At Clinica Amistad, a non-profit clinic in Tucson, Arizona providing free primary care healthcare services to uninsured individuals, the opportunity exists to increase the number of patients referred to a Certified Diabetes Educator. Methods: A non-randomized, uncontrolled before and after study design was used to assess the efficacy of a diabetes flowsheet implemented to increase the number of provider referrals to Certified Diabetes Educators for patients with Type 2 Diabetes. Pre- and post-surveys were administered at Clinica Amistad to assess provider and PCP helper perception of the ease of use and usefulness of the flowsheet. Appointments scheduled with Certified Diabetes Educators were counted for the periods of time prior to and after the flowsheet implementation. Results: Pre- and post-surveys were collected from four providers and six PCP helpers which reflected a favorable perception overall of both the ease of use and usefulness of the flowsheet. Data were collected from 27 flowsheets, showing six patients had seen a diabetes educator prior to this project, 11 patients were referred to a Certified Diabetes Educator and 10 patients were not referred. When appointments with diabetes educators were counted for both the five-month period in 2017 and 2018, the number of appointments scheduled with diabetes educators increased from the previous year by 118%. Conclusions: These results are an encouraging initial indication of the efficacy of a flowsheet as a tool to increase referrals of patients with Type 2 diabetes to Certified Diabetes Educators in a local free clinic.
