We are pleased to inform you that our Medical Clinic is now accepting medical reimbursement request for 4th Quarter

For your guidance, please see our processing schedule for 2015:

QUARTER Deadline of Submission Crediting Date

Second Quarter June 25, 2015 July 3, 2015
Third Quarter September 25, 2015 October 5, 2015
Fourth Quarter December 10, 2015 December 18, 2015



Every quarter, regular employees can reimburse up to Php 2,500.00 or the equivalent amount of their earned entitlement (whichever is higher). Budget allocation will be carried over to the next processing period should the employee defer from using the benefit this quarter. The Medical Reimbursement Allowance for employees who were regularized after January 2015 will be pro-rated. Benefit will depend on their regularization month (i.e. if regularized in February, MRB = Php 833.33 x 11 or Php 9,166.67).

Kindly submit your accomplished Medical Reimbursement Form and Official Receipts to our Medical Clinic c/o Mark, Vlad, Grace, or Brigette. Valid receipts are from January 2015 to present.

Please be reminded of the following guidelines before you make your purchases:

1. OTC medicines, vitamins and medications for hypertension, diabetes and the likes may be reimbursed
2. Herbal Teas, health and dietary supplements such as Eye Berry, Arthritis¸ and the likes, are not reimbursable.
3. Ointments and treatments for aesthetic usage, i.e. scar and warts removal, varicosity and cracked heels are not reimbursable. To be eligible for reimbursement, some treatments, prescription drugs or services that are deemed cosmetic in nature must be supported by a medical certification from a health care provider.
4. Items for first aid such as gauze, microphore tapes, surgical gloves, injections and the likes are not included.
