have been granted another passport after this passport was issued.
Please send me the passport issued on 08/12/2000 for cancellation.
If your previous British passport has been lost or is not available please complete the enclosed
form LS01, (unless the passport was an old blue style passport issued before 1992 or a temporary
visitors passport, obtained through a Post Office. In which case you should let me know).
